Need a form for a dispute of charges

I was double billed over two years and I want my money back. I am owed 4116 dollars and over the past 4 months I have gotten the run around on this problem

Customer service wanted to charge for 4k receiver and charge monthly fees plus maintenance tech cost. Then came back and informed me that all fees would be eliminated. The new catch is I would have to sign 2yr contract. I declined and advised them I would check other providers, I have been a loyal

I have been logged out of my Max Streaming APP and I cannot log back in. I'm receiving an error message that my account doesn't have the subscription but that's incorrect. I pay for HBO/MAX through Directv. I have not been able to log in for days and was in the middle of a show on Max!

I searched but didn't really get a concrete answer, I have satellite based service with the DTV app, when we are away at beach I can watch the dtv app on firetv, my question is am I able to record while away from home via cloud? Or is it just able to record through my homebased DVR's?

I have a HD DVR, Model HR21-100 that is stopping and skipping. I have had it for 17 years. If I ask Direct TV for a replacement, what model will they replace it with?  How much will I have to pay (or do I have to pay) as the dvr is leased from Direct Tv?

I plan to buy a Samsung QN65S90DAFXZA OLED 4K UHD Smart Tizen TV and need to know which receiver I need.

I keep getting no video signal on one of my tv and can’t fix it.
