Agreed. If you were sent an order confirmation through email it should be included in there as part of the work order. Otherwise, after installation, as stated by teetertotter, you should be able to follow-up with Costco regarding the card. Just ensure that the card was indeed included with the order before proceeding. Many times, there were issues where the card was not applied properly and thereafter you never actually get the gift card.
Be aware you ordered DirecTV through the 3rd party retailer who leases space, not Costco itself. Usually that is Smart Circle. Make sure to read through the order confirmation DirecTV sends you before you are installed to verify all is as expected.

In other words, you go to the Costco website, click a link that takes you to the Directv website, and complete your purchase there. It was not a 3rd party or in-store purchase. My order email confirms that a Costco reward card is included, but I never received one either. It's been more than 4 months.
File a BBB complaint with the 3rd party. I would verify with DirecTV the dealer on the order as Smart Circle may now have space on Costco's website to do the same as in-store. In either case a rewards card from the 3rd party (usually store specific) is not the responsibility of DirecTV, as that is the 3rd party's offer on top of the DirecTV one.

Smart Circle is the dealer that leases space in the Costco store. It is possible they are 'leasing' space on Costco's website. I would verify with DirecTV to be absolutely sure if Costco or Smart Circle (or even someone else) counts for the dealer order.
Just want to make sure with any resolution you seek (BBB complaint or otherwise) that it goes through the right channels.

Make sure the reward offer is part of the order confirmation, if it's not in there then you Won't get the card no matter what
