9 months after preventing all PPV purchases from house Kid was able to rack up $140 in PPV using remote for old batman episodes

Issue:  In Janurary 2023 a purchase was made by my child.  I spent an hour on the phone back then working on a credit because prior to that another purchase had been made and I was told by the support agent that they took care of preventing that in the future unless my wife or I called into DTV specifically. 

So today I get the bill and I am shock that there are PPV and BUY Purchases listed on the bill for over $140.

I call into Customer Service and they proceed to show that these were purchases by the remote however we continue to go in circles regarding that it was all suppose to be blocked 9 months ago by the agent.  I ask for a supervisor and he offers a credit of $50 and says that he has blocked it again.  I asked about the remainder and he says he will try some more.  He got it up to 101.99.  Still leaving me for over $40 in purchases.  I also do not know how my child did this when the spending amount was set to $0.00

Why does this continue to happen every few years.  One time I was told that their was an update to the receiver and that erased the block.

Kid is 12, and is autistic.  We have powersupply that is set with a timer that shuts down the receiver and wakes up the receiver at certain time.  I have parental controls configured.  Kid is at school but yes will be talked to this evening after getting home.
