Reward Card of $200

I was told that I would receive a reward card of $200.00 for signing up for Direct TV Streaming and At&T internet.  I was also told that the packages included Christian TV programming, but after signing up, I found out that there was absolutely no Christian TV channels.  It has been two months and have not received any rewards cards.  Please respond ASAP.
