Money back from direct tv

My account was closed with dtv owing me money back which they provided me with a prepaid mastercard which of course has fees and other associated (Edited per community guidelines). At the bottom of the paper the card is attached to, it says you can go online to request a paper check or move funds to your u.s. bank account. The only problem is it doesn't give you a we(Edited per community guidelines)ite address to login to. What's the we(Edited per community guidelines)ite to get my money?
