Credit Balance of closed account

I received a debit card with a balance of $59.22 which represented my credit balance when I terminated my DirecTV account. At the bottom of the letter it says if I want my money in a paper check form all I need to do is go online and request it. Does not give a link or a phone number. This card if I activate it cannot be used at ATM’s and says it has a $3 per month fee. I don’t want it, I want the described paper check but still have no idea how to request it. My former account was made zero and closed. How do I request the paper check that any other normal company would have issued to avoid situations like this!

Most likely that is referring to contact the debt card company for a check.
