The NDR (non destructive disaster recovery) will rebuild the indexes to the content. However, the file system isn't a general purpose one and isn't supposed to really need defragging.
I may be asking for trouble by saying this (knocking on wood now), but I've had a DVR for 12 years that gets moderate to heavy use and I've never done an explicit defrag operation on it. Though I have done an NDR. Many users ago.
Rebuilding the indexes is probably a step in the right direction. I recently deleted a bunch of my recordings to create more available space on that hard drive. Since then, I've noticed that recordings play less smoothly and there are brief halts and skips as it plays. And when I refer to "recordings", of course that also includes "live programs", of which as much as one hour is always being recorded so we can "pause live programs".
Anyway, I suspect a good defrag of the hard drive would clear up these issues, but I've never known of any way to do that. Maybe a reorg of the indexes would be good enough.
There have been a number of threads on how to do it. Look in this one. Hopefully you’ll let us know if the procedure resolved the issue you’ve been facing.
In the post gr8sho references, in step 3 it's called "Non-Disaster Recovery". It should really say "Nondestructive Disaster Recovery". What they term "Disaster Recovery for DVR/Receivers" in the second part should be called "Destructive Disaster Recovery"
The obvious difference between NDR and DDR is the loss of your content.

Doing some research into how uverse system operates, it seems defragmenting isn't possible using the windows CE operating system. Which apparently is a bit of an issue eventually with hard drives for our dvr. Probably explains why as the hard drives get older since last destructive recovery, they seem to get buggy and require more and more free space to work correctly. My guess is when they are too full a single long recording might be fragmented dozens or hundreds of times. 10% of a 1 tb drive seems to be the border for perfect operation when the drives are midway through their life. but right before my recent one died I would've needed at least 25% to have perfect operations again.

Why do you think this is needed? There is no access to any of that type of capability.
You can perform a non destructive factory reset if you think there’s some issue that can be cleared up.
