Direct tv says stream over internet to multiple TVs is extra fee. How much

Not quite sure what you are referring to, but here is the DirecTV Stream policy for the amount of units you can stream to, with a subscription.

I don't think that's true. But you're limited to 3 out-of-home streams vs. 20 in-home streams.
