The network has inserted a spot where the service provider (in our case DirecTV Stream) can insert a commercial, but none has been sold, by DirecTV, for that spot, so we see that insert.

Agreed and understood on the reason for the break screens (unsold time).
One wrinkle, however. We have noticed that when we have the same program on two TVs or devices, very often one will show the “break in progress” screen while others show commercials. Once the program resumes, all are back in sync, so it’s not a matter of devices being behind or ahead of live TV. It also varies between devices. Sometimes, the wired TV with the DVR shows the break screen and sometimes another device has it while the wired TV is airing the commercial. There is no discernible pattern.
With a prior streaming service, we also noticed that two devices would show entirely different commercials at the same time during the same program. It’s all very odd.

My guess to that would be that on the DirecTV Stream device it is a spot that DirecTV could sell for an add, and if your other other device was a Roku or Fire Stick it could be their option to sell a spot. One sold the spot and the other one didn't.

Aha! Yes, the second TV has a Fire Stick. That must be it. Thank you.
