how to add an "add-on channel" to directv stream?
I have the Choice package. When I look at the channel lineup matrix, I see that some channels are listed as "add-on" channels, with a blue pricetag/dollar sign icon in the table. I assume that means I should be able to subscribe to that channel (for a fee) as a single add-on to my Choice lineup, rather than upgrading to the next lineup package deal (the "add-on" channel of interest is listed as an add-on channel in all 4 package choices, so it is apparently not included in any of them). My question: how do I "add" the "add-on" channel to my subscription?

Which channel? You can also make changes to your subscriptions on their website under "account details"

So MTV is included in the entertainment package, but MTV Live is not. Can I pay a fee to add that channel to the Entertainment package?

No. You would need to upgrade to a package that includes that channel.
